The 2027 Star Ratings results released by CMS earlier this month send a clear message to Medicare Advantage plans: maintaining strong performance is getting harder. Fewer contracts achieved 4+ stars, nearly half of all cut points became more difficult, and the gap between lower and higher Star performance continued to narrow.
But the 2027 results are only part of the story. With the Star Ratings program shifting greater weight toward clinical outcomes and member experience, health plans need to understand not only what changed this year, but what those changes signal for the years ahead.

- The Star Ratings formula is shifting toward clinical outcomes and member experience. As measures retire and weighting changes, HEDIS®, HOS, improvement, and pharmacy measures are projected to account for 80% of rating weight by Star Year 2029.
- Plans need to prepare for continued change. With guardrails delaying some cut-point increases and more measure changes on the way, success will depend on forecasting performance, protecting heavily weighted measures, and monitoring member sentiment throughout the year.
